Practice Starts

 Practice starts on Monday February 23, 2009
6:15-8:30pm at the Westview Stadium

Parents & Players:

 We are glad you are interest in becoming a member of Westview Girls Lacrosse.  Practice starts on February 23, which is only three weeks from now.  In order to practice we need you to complete these forms: “Westview Girls Lacrosse Registration: 2009”, Beaverton School District; “Athletic Participation Information”, “Player Code of Conduct”, “School Sports Pre-Participation Examination”, signed by your physician and the fee of $350 or call me regarding a payment plan.  If you are a freshman and this is the first sport played at Westview you need a physical.  If you’ve played at Westview before, your physical is good for only two years.

I need the forms no later than Friday, February 13.  I need to make copies so the coaches have some of the information with them at all times, the balance gets delivered to the  Athletic Department along with their fees.
